Yep that's what I did. I doubt they're planning on us finding cardstock and figuring out how to print those on them without any specific directions

A copy got sent to both my immediate boss and my corporate office. I assume the same questionnaire. It had items that dealt with your clearance. Said something along the lines as official government document. Asked some no brainer questions similar to the e-qip. 1 page, front and back, looking to verify employment. Asked if the individual in question had any history or trouble with the law, alcohol, drugs, etc - and if they'd vouch for the individual in question's character for a clearance. Again it was real no brainer questions, but man if your employer wanted you to stay he could screw you over real quick like.

thats what it says under tips on the eods website
I saw that, I was just seeing anyone knew the actual reasoning behind it.

One thing I might mention as well, it did come with a prepaid envelop for your employer to return send it. Might be good to tactfully make sure your employer actually sends it off and doesn't sit on it for a month and half. Just a thought.
I wasn't unapproved yet, still says "submitted" I guess I am just stressing over something I had to disclose to them that happened when I was still a minor.

HR won't deny you for that... That'll be the OPM and your background check. I would say the only way to get denied on your EODS forms is if you fill them in incorrectly

Yeah, I printed it on normal paper and put "Air Traffic Control Specialist Trainee" like it said in the TOL email
One of my EODS wasn't accepted because I put my phone number in the Mobile space and not in Day or Night number, so it didn't show up on one of the forms.
My HR rep sent me an email & the way to fix it. I submitted it the next day, it's still waiting for approval.
I just got a CIL today, so who knows.

Took me 5 minutes but I have no medications so I imagine it takes longer if you have medications or a condition.
