MMPI-2 Tier 2 Process

Was just asked to write a personal statement on range of motion for an injury I have. Is there any specific way to do this or format? Or just a detailed paragraph and then just my signature?

I just had two send in two personal statements for traffic tickets I got, just be very detailed and say what you have too don’t leave anything out. Then put your name and the date on the bottom. I usually address it to whom it may concern.

Man that's awesome. They made me pay out of pocket for a physical therapy report on mine. If I were you, I'd read about the range of motion portion of a Functional Capacity Evaluation and replicate the information in a paragraph form with your own angle measurements-- that might be trying too hard, but better safe than sorry maybe

Yeah lol, one was over 4 years ago. A careless driving ticket for passing someone up illegally the other because I didn’t have my front plate on my car. He wanted two personal statements. No one to blame but myself. ??

That’s fucking weird. I have had a million tickets. No one has once asked me about them.

Edit: Also, front plates are lame. Thank a God I can just keep my Florida plates when I move.

Count your blessings, now I have to go through this to get my MMPI appointment done. For some reason RDC has stopped replying to emails, he usually replies very promptly. I’m kind of worried because people on here saying that they’re talking to him. I just want to know what’s going on with my situation.

Don’t feel bad, he’s been MIA for me for 2 months. I emailed him yesterday but still nothing. Oh well.
 
I found that when he wouldn't answer my emails, I would also leave a message on the medical line. He would then end up responding to my email and the VM around the same time

I had to go way back in my email but im almost certain its this one. It will give you an option to leave a VM, then RC should respond to you via email within 7 days.
 
Anyone who already did their evaluation waiting for their results?
