I've only been asked for information pertaining to conditions that need a special issuance. Pharmacy records going back 8 years, doctors notes regarding to the conditions specifically. That's about it. If they need more paperwork, they'll ask. If you have any questions once you hit that point, call the person that sends you the email asking for more info. They'll be able to clear it up for your specific case way better than any of us could :)

Like for pharmacy records would that mean calling up your local Walgreens or wherever you got your meds? Lol

Sorry for all the questions, really trying to figure out what I can get done first and get out of the way while I wait for the email.

Looks like it's best to just wait for the email telling you what extra stuff you should send back and that will tell you what to get and how to get it?

Yeah, that's what I'd probably do.

Just be prepared to fill out a lot of paperwork to get your records from these folks. And making a lot of phone calls to hunt down who has what.
 
Got my fingerprints taken but they typed in my height/weight/hair color/DOB when the FAA form says specifically to do that part in black ink...? Also do you write N/A for aliases or just leave it blank?

Well I finished my e-qip just now. So now all I am waiting on is my reply from medical with my medxpress information. Hopefully everyone's appointments go smoothly. GL again everyone.

I'm actually burning some vacation days at my current job just to get all this paperwork over with.

If they typed it, wasn't it in black? and don't you use ink to print it? I bet that's okay.

Same boat. Got my CIL at 11:45am on Friday. I'm guessing most of my contacts were wrapping up for the weekend. I know my paperwork is going to take a while, I just want to get started.

I'm curious about my MMPI-2 as well - I'm about 3 hours away from the nearest FAA facility, and my POC is located about 4 hours past that. I just want to know what kind of drive to plan for. I think I'll just call the number on Monday if I haven't heard back.

I emailed my poc for the MMPI-2 and haven’t heard back. I’m gonna call them Monday, and also co-ordinated with my wife and when’s best to schedule a medical exam. Mines special though, I had lasik but my right eye isn’t there yet but too soon to do an enhancement. The place I had it done said they’d write me a prescription so I can pass, but then I’ve gotta get those filled and so on and so forth.

So yes, I’m in the same boat, jelly of all those finished already and still waiting to finalize appointments.
 
1. Obtain two sets of fingerprints. The SF-87 (Fingerprint Card) and instructions are attached. You may print two copies of the attached SF-87 or can use a law enforcement agency provided card (two cards) to have your prints taken. We accept “ink prints” and “digital/electronic” prints. If you have your fingerprints taken digitally, please ensure the agency can print them on card stock paper. Ensure all required information is completed on the card for you and the finger-printing official signs both cards. If you are located near an FAA facility that provides fingerprinting services, you can arrange to have your fingerprints taken at no charge.





2. Print, complete, and sign the Disclosure and Authorization Pertaining to Consumer Reports, DOT Form 1631 (attached). Note: If you have “frozen” your credit report you must unfreeze it so that your background investigation can be conducted.



3. MAIL (FedEx/UPS/USPS) the following original forms to the Security POC listed below:



(X) DOT Form 1631

(X ) SF-86 Form for National Security Positions.

(X) SF 87s (2) Fingerprint forms



4. Access the Electronic Questionnaires for Investigations Processing (e-QIP) website at National Background Investigations Bureau and complete the below identified questionnaire. A Registration Code is required to access e-QIP. Your required registration code is -----------------.



5. After you have completed the eQIP questionnaire:

ü Digitally sign the signature pages (forms will automatically upload to e-QIP)

ü Release the questionnaire

The (SF86) looks identical to the E-QIP questionnaire from a quick google search and is a 127 page document. No SF86 was attached to my email. Also I did not see any remarks in my email about waiting to release E-QIP until after mailing as someone else previously mentioned.

Strange..... My letter does NOT say that. Mine says that the Registration Code is required and will come in a separate email. Then it says DO NOT RELEASE the e-QIP questionnaire UNTIL AFTER YOU HAVE OBTAINED AND MAILED YOUR FINGERPRINTS.
It says nothing about the Security POC receiving the fingerprints/documents first.
 
Just a heads up, if you haven't scheduled your MMPI-2 yet, make sure you were referred to the right POC. My test ended up being scheduled in Anchorage. I live in Southern California. My POC's email signature said something about Seattle/Alaska district but I didn't think anything of it. I guess it did matter. Clerical errors happen, save yourself some time and confirm you're talking to the right person?
 
The e-QIP is an online platform that allows applicants to fill out the SF-85, SF-85P, or SF-86 as indicated by the agency sponsoring their application. The SF-86 is filled out entirely on the e-QIP. I suppose you're welcome to download a fillable PDF ahead of time to get your responses in order but that's all you'd do with it.

Strange that some people are getting told they won't get into e-QIP until their fingerprints are in and others are saying mail them, then release. Different regions do it differently maybe?

edit: also you DO NOT have to unfreeze your credit report if you have it frozen (and since last September it's free to freeze it so you really should). See FIN 19-01. My security POC confirmed this.
