It does not take 13 months to get through the Academy unless you are hit with massive delays. I believe during the height of COVID there may have been a few people that got close to the 13 months, but that’s extremely unlikely at this point. Even with a COVID delay or two, you should be finished within 5-6 months at the max at which time you will either be terminated (if you fail) or offered a permanent position at a facility (if you pass).

Not typical. Under normal circumstances your temporary employment begins when you get to the academy where you get onboarded into the FAA. The academy is 3 months for terminal and 4 months for enroute.
MMPI is the psych eval. Has nothing to do with your health. There is a medical eval that you have to take and pass but it's not very stringent. They do an eyesight test, hearing test, check your blood pressure etc
If you have a smartphone, then go to the app store and download a scanner app. Then, take a pic of the filled-out form and email it to HR. You won't have to convert anything since most apps will give you the option of taking the pic in JPEG or PDF form.
