I was recently reminded this existed after completely forgetting about it. Its now been updated to have current facility level information.

I noticed that the exe takes several minutes to open on my laptop after the CMD window opens. I'll be looking into this in the next few weeks to see if I can fix that, or I'll at least try to make the logging better so its clear that the program is still loading.

Leave all the points blank. They aren’t used anymore. If you’re worried about messing something up, just ask HR if they need anything else or if you’re all good.

Nope. Just your signature required.

Only your signature is required.

Idk if the resume is required (probably not) but I'd attach it anyway cause if HR thinks you're missing documents it could just be a hassle 🤷‍♂️

Whenever i try to run the program it says "ERROR: 1. Personal Information.xlsx not found! Aborting!" I have the filed out personal information in the folder in which everything else is. Can someone please help me figure out what I'm doing wrong? Do I somehow attach this into the program?

I believe I'm following the instructions doc word for word. I just do not know what I'm doing wrong.

UPDATE - I'm able to get it done for one of five facilities, but not all information from the -43-1 is compiling into the paperwork correctly. Meaning its leaving blanks on specifically the type of facility I've had cert's (tower and location), and my cell number.
