So im doing the medical part of the CIL. The medical form says a medical condition is where you had to go see a medical professional, does this mean even if you were to go into an ER for an accident. Not a condition that impedes day to day work? or just visits to the doctors for underlying medical conditions? thanks guys

I did mine at ZLC and the main contact scheduled while I was on the phone. Took only a few minutes.

When I went to schedule my MMPI, I tried Sky Harbor. I was told they were not, and to try Mesa Airport. That's when we got thr inevitable delay order, but then a week later the gal from Deer Valley Airport emailed me and said she was trying to help the proctor at Mesa and she could see me in mid Jan. That's where I took the MMPI.
Not sure if something changed where they cant do it now, but DVT certainly accepted me a couple weeks ago.

Falcon field airport. People replied very quickly.
ZLC had a wait time of like 2 months because they said they could only do it on Mondays. I ended up driving to KBOI. they were able to get me in the next week.

Not sure where you're located or willing to travel but for my two cent contribution, I went to KPOU and they had immediate availability at the time I scheduled mine. Since that a few more people had scheduled but it didn't seen like they were backlogged or anything.

I went to Albany in December. My appointment was originally cancelled because I wasn't a priority, then a week later the proctor e-mailed me back saying we were on again and I could have the original date and time I set up, which was 3 days away. He told me people were coming from all over the state to take their MMPI there, and the day I took it someone drove up from LI.
Who knows now being almost two months later, but I'd call everywhere you can just to get it done.
If you guys couldn’t find out contact information for supervisor from previous jobs, what did y’all do? My POC is telling me to look them up online and find the info. Problem is I only remember there first names. I worked at 2 grocery stores for 3 months each 5 years ago. They probably don’t even remember me.

There were 2 jobs where I hadn't the slightest clue who my supervisor was. So instead I put down the name of someone I worked alongside and put a note in the comments. You could always call the store and see if they work there still and get their info that way.
I took my MMPI at Falcon Tower in Mesa. Leanne Mayr. She was very helpful and got the test done in no time
Also, do I have to get them done at a law enforcement agency or can I go to a place like PrintScan.
The e-mail says:
"To avoid any delay in the processing of your background investigation, you are strongly encouraged to use a local law enforcement agency or other third party fingerprint service provider to have your fingerprints taken."
But the fingerprint form says:
"Fingerprints may be taken at any local law enforcement agency or a Federal Aviation Administration Regional Security office. They may also be done at a military installation should you have access to one."
Call your local Trooper barracks' non-emergency phone number and ask. They don't have the ability to print the digital scans but can do it on a DCJS ink card; you'll just have to add the section for scars\marks\tattoos which isn't a big deal.
If not, the FAA regional office is in Queens. That's not a far drive for you.
