Exactly. I basically had a folder on my desktop that said err and had everything filled in on all the forms/cover letters etc but facilty name and region and my signature, as well as a PDF of my eval etc. makes refiling or submitting new ones a 5 minute thing.

If you are too Lazy to do that you don't want it bad enough to be complaining.

If you apply to management positions, only the applicant's signature is required. I always thought that was strange.

The page (4) your rating official signs can just be printed separately. Then you can change the other page (3) that shows what facility you’re applying to. Only one signature needed for an unlimited number of facilities and no white out necessary. Of course I will not comment on the legalities or condone such unscrupulous activities.
